25A. Application for duplicate licence
(1) The Commission may, on an application made
by a licensee, issue a duplicate licence where the original
licence has been misplaced, lost or destroyed.
(2) An application made under subsection (1) shall
be accompanied by –
(a) evidence that the misplacement, loss or
destruction of the licence has been reported
to the Police at the earliest opportunity;
(b) such fee as may be specified in
the FSC Rules; and
(c) such other information as may be required
by the Commission.
