10. Record of patient (1) Every centre shall keep a complete record in respect of each patient or security patient. (2) Every medical officer, psychiatrist or nurse attending to a patient or a security patient shall enter into the patient’s record all matters that concern the patient, including those necessary to enable the Board and the Commission to have adequate information about the patient’s mental condition, any treatment administered, any refusal of examination, treatment or admission.
