13. Re-application for licence
A fresh application for a licence shall be required in the case of—
(a) the proposed use of different or significantly modified premises
or sources;
(b) the proposed use of different radioactive substances, radioactive
materials or larger quantities of such substances or materials;
(c) a change taking place at the site of the premises or their surroundings which is likely to affect radiation protection requirements; or
(d) the death of a licensee.
